
To be eligible for PACE, an individual needs to be:
- 55 or older
- Certified by the state to need nursing home level of care – have a chronic illness, disability, or be in need of assistance
- Able to live in the community with PACE support without jeopardizing your health or safety
- Live in one of the following zip codes: 70001, 70002, 70003, 70005, 70006, 70032, 70043, 70053, 70112, 70113, 70114, 70115, 70116, 70117, 70118, 70119, 70121, 70122, 70124, 70125, 70126, 70127, 70128, 70129, 70130, 70131.
Steps to Qualify:
- Initial Qualification Over the Phone
- Home Visit by the Intake Coordinator
- Level of Care Assessment
- Collection of Financial Information
- Medicaid Application
- Visit to the Center
- Potential Enrollees are asked to arrive at the center at 9:00am the day of their scheduled visit. They will visit with the PACE staff, and will have an opportunity to interact with current PACE participants. Lunch will not be served.
- The Interviews include visits with:
- The Doctor
- The Social Worker
PLEASE NOTE: PACE services will not begin until the FIRST DAY of the month FOLLOWING the enrollment meeting.
Potential enrollees with dementia or Alzheimer’s should have a Power of Attorney in place in order to join PACE.
